Before your child can begin at MSLA, all required enrollment forms must be completed and returned in accordance with the Massachusetts Department of Early Education and Care (EEC) regulations. These forms help us ensure your child’s health, safety, and well-being while in our care.

All enrollment forms can be downloaded and returned to the center upon completion. Families may also request a printed hard copy enrollment packet to pick up in person.
